2 more on top of five?

When the specification says:

After ordering: After ordering in a restaurant, customers expect food within 4 ticks but wait for up to 5 ticks for their food to be SERVED. If they haven’t received food until then, the group leaves immediately. If at least one person on the table has received their meal, they wait for 2 more ticks.

is it 2 more on top of the 5 initials or 2 more after the first person was served?

Our tutor told us it’s 2 more ticks after the first person was served.

So if they order at tick 4 and the first one gets his food in tick 5 they wait until tick 7, instead of 9 or 11?

No. They wait until tick 10. The ordering tick counts as the first waiting tick, so the original five-tick window is ticks 4 to 8. The partial serving adds ticks 9 and 10.
